disabilities. Through collaboration, they empower individuals to age with dignity and independence at home.
Driven by resilience, dedication, and a commitment to high-quality care, these organizations,
led by ONECARE Home and Community Support Services, are transforming challenges into opportunities
for better health outcomes.
Addressing the Challenges
At 80 years old, Mary faces significant hurdles in accessing the care she needs. As a devoted nun in the Huron Perth region of Ontario, she relies on community support services. However, with no family nearby and multiple organizations operating independently, navigating a fragmented care system becomes challenging. This often forces her to repeat her story to different service providers, leading to frustration and a lack of cohesive support.
Mary's experience is not unique; many seniors face similar challenges, resulting in poor experiences and unmet needs.
A Digital Transformation Journey
To tackle these challenges, 13 home care and community support service organizations have partnered with INVORG to embark on a transformative digital journey. This collaboration has resulted in the Client Centered Care Platform (CCCP), also known as Total Health Care, designed to place clients at the heart of the organization and its services.
